Care continuum to monitor and control chronic pain in oncological patients: an Italian experience of integration between hospital and territory

ABSTRACT

Since in our community there was no chronic pain management protocol in place, we decided to set up and implement a chronic pain management guideline. The objective of the study was to improve the assessment of chronic pain in oncological patients using systematic monitoring with the Numerical Rating Scale (NRS). Our research team elaborated the management guidelines and the operational ways: informative brochure, diary of the pain, nursing pain schedule, productive factors schedule. Patients in the study were identified in the hospital and in the territory. Evidence of pain evaluation were found in the 100% of the cases for the hospitalized patients and in the 80% of the cases in charge to the Day Hospital.
